Getting Connected is Simple
Your healthcare provider, pharmacist or case manager can enroll you in the FUZEON Nurse Connections program—or you can enroll yourself, with the approval of your healthcare provider. Enrollment forms are available at your doctor’s office, pharmacy, AIDS Service Organization, this Web site or call 1-877-4FUZEON to enroll over the phone. Once the form is completed, we will work with your doctor to arrange the first call from a FUZEON Nurse Educator within 24 hours. No other paperwork is required.

Once your healthcare provider helps arrange your first visit with a FUZEON Nurse Educator, you decide when and where you would like to meet. You also decide how much follow-up you want. The services are there for you when you need them—at no cost to you.
