Getting Connected is Simple
Your healthcare provider, pharmacist or case manager can enroll you in the FUZEON Connections™ program — or you can enroll yourself. Enrollment forms are available at your doctor's office, pharmacy, AIDS Service Organization and this website. Once the form is completed, we will work with your doctor to arrange your first visit from a FUZEON Nurse Educator.

You can enroll in FUZEON Connections over the phone, as well as get answers to your questions about FUZEON, when you call the FUZEON Connections Center. This toll-free hotline is staffed by FUZEON Nurse Educators.
Choose the level of support you need
Once your healthcare provider helps arrange your first visit with a FUZEON Nurse Educator, you decide when and where you would like to meet. You also decide how much follow-up you want. The services are there for you when you need them — at no cost to you.
